Privia Care Partners works with independent primary care physicians to help them coordinate improved care for their patients. We provide tools and resources to practices we partner with and serve as an extension of the primary care office by working with patients in proven care coordination programs. Privia Care Partners participates in the Medicare Shared Savings Program, with partners across the country.
You may have received a call or letter from one of our dedicated care team members. Their role is to work with you and your primary care provider to help you take charge of your health and wellness. Our care team is made up of registered nurses, social workers, community health workers, and support team members who can proactively help you navigate healthcare challenges and meet your care goals. Our care management programs are available at no cost to patients and do not require additional office visits. We know health care can be complicated, especially for individuals with chronic conditions.
